Treatment Foster Care Case Manager

Location: Boise, Idaho; Nampa, Idaho; Caldwell, Idaho; Garden City, Idaho; Meridian, Idaho; Star, Idaho; Kuna, Idaho; Payette, Idaho
Position Type: Full-time 
Remote Status: Hybrid / Community-Based
Salary: $20–$24 per hour

About the Role

Clarvida is seeking a Treatment Foster Care Case Manager to support youth with significant behavioral, emotional, or medical needs in a family-based treatment setting. This role is an essential member of the Treatment Team, coordinating behavioral health and community-based services to help youth stabilize, achieve treatment goals, maintain placement, and work toward permanency with their families.

As a Case Manager, you will collaborate closely with foster parents, clinicians, the Idaho Department of Health & Welfare, and birth families to develop individualized treatment plans and ensure consistent progress. This position offers a flexible schedule, supportive team environment, and strong opportunities for professional growth.

Responsibilities

  • Develop individualized treatment goals and service plans for children and youth in Treatment Foster Care.
  • Coordinate behavioral health, educational, and community-based services.
  • Monitor treatment progress, update goals, and support readiness for discharge.
  • Provide regular communication and support to foster parents.
  • Partner with birth families as appropriate to support permanency planning.
  • Maintain accurate documentation and timely progress notes.
  • Participate in clinical supervision, team meetings, and ongoing training.

Required Qualifications

  • Bachelor’s degree (Human Services field preferred): Social Work, Psychology, Sociology, Counseling, Family Relations, Nursing, Occupational Therapy (OT), Physical Therapy (PT), Therapeutic Recreation (TR), Speech-Language Pathology (SLP), Behavioral Sciences, Art Therapy, or related field.
  • Ability to pass background checks, including Enhanced Fingerprinting with Idaho DHW.
  • Valid driver’s license, reliable transportation, and proof of auto insurance.
  • Experience with EHR systems is a plus.
