About the Role
Clarvida is seeking a Treatment Foster Care Case Manager to support youth with significant behavioral, emotional, or medical needs in a family-based treatment setting. This role is an essential member of the Treatment Team, coordinating behavioral health and community-based services to help youth stabilize, achieve treatment goals, maintain placement, and work toward permanency with their families.
As a Case Manager, you will collaborate closely with foster parents, clinicians, the Idaho Department of Health & Welfare, and birth families to develop individualized treatment plans and ensure consistent progress. This position offers a flexible schedule, supportive team environment, and strong opportunities for professional growth.
Responsibilities
- Develop individualized treatment goals and service plans for children and youth in Treatment Foster Care.
- Coordinate behavioral health, educational, and community-based services.
- Monitor treatment progress, update goals, and support readiness for discharge.
- Provide regular communication and support to foster parents.
- Partner with birth families as appropriate to support permanency planning.
- Maintain accurate documentation and timely progress notes.
- Participate in clinical supervision, team meetings, and ongoing training.
Required Qualifications
- Bachelor’s degree (Human Services field preferred): Social Work, Psychology, Sociology, Counseling, Family Relations, Nursing, Occupational Therapy (OT), Physical Therapy (PT), Therapeutic Recreation (TR), Speech-Language Pathology (SLP), Behavioral Sciences, Art Therapy, or related field.
- Ability to pass background checks, including Enhanced Fingerprinting with Idaho DHW.
- Valid driver’s license, reliable transportation, and proof of auto insurance.
- Experience with EHR systems is a plus.
