An exciting opportunity has become available to work within the Outreach team focusing on the needs of complex patients within a care facility and/or their own home.

The work is hybrid but travel between HQ and patient locations or care facilities may be required. 

 

The post holder is an experienced clinician (Nurse, Paramedic, or other Health Care Professional) who is accountable for the identification, coordination and management of complex callers and care facilities within their geographic area, working closely with their local and/or regional multidisciplinary teams.

The post holder will have a key day-to-day advisory and investigative role as a specialist advisor in Care Homes and complex callers.

They will also be responsible for developing and maintaining key relationships with internal and external stakeholders including, acute and primary care providers, community-based teams, NHS 111/OOH providers, mental health, social services and Police.

Working alongside the clinical area teams, clinical pathways team, and external agencies to identify unmet needs and appropriate pathways for these patients.

Person specification

Criteria

Essential criteria

  • Registered Health Care professional (Level 1 Registered Nurse, Paramedic, OT, Physio, Social Worker etc.)
  • Significant post registration experience in pre- hospital or community setting
  • Effective engagement and influencing skills, with an ability to negotiate and influence people with tack and discretion, leaving individuals satisfied with outcomes whilst maintaining Trust policies and procedures
  • Competent in Microsoft Office applications including Word, PowerPoint and Teams

Desirable criteria

  • Understanding of wider healthcare community services and the interface with emergency care services
