Are you a dynamic Care Coordination expert who thrives in a fast-paced, high-autonomy environment and has a passion for empowering frontline teams through education and innovation? This is your opportunity to make a system-wide impact across Banner Health's acute care facilities throughout the greater Phoenix area — West Valley! In this exciting  role, you will serve as a trusted subject matter expert, educator, and change champion, supporting facility and system leadership by onboarding new hires, delivering one-on-one and large-group training, diving deep into data auditing and reporting, and problem-solving alongside frontline staff, associate directors, and directors. No two days look the same — you will manage a dynamic schedule with varying deadlines, cross-cover multiple hospitals, and engage with a wide range of internal and external stakeholders, all while operating within a matrixed leadership structure with a remote direct leader. The ideal candidate is a self-starter with exceptional time management skills, a calm and solution-focused presence in the face of change, and the ability to not only master new processes quickly but turn around and teach them in a way that inspires confidence and unity across the care coordination team. If you bring strong presentation skills, a collaborative spirit, a positive attitude, and a genuine commitment to elevating the care coordination experience — this role was built for you!
