Cardiometabolic Health Program Lead
Be part of a transformative effort to improve the health and well-being of rural communities across Indiana.
Our client is leading Indiana’s 5-year Rural Health Transformation Program, including the Make Rural Indiana Healthy Again Regional Grants, a historic $600M investment to strengthen healthcare access, prevention, and system resilience across 64 fully rural and nine partially rural counties. This initiative brings together state leadership, healthcare providers, and community organizations to design and implement solutions that reflect local needs while advancing a shared, statewide vision for better health outcomes.
This role leads the design and statewide implementation of cardiometabolic health strategies that directly improve prevention, early detection, and long-term outcomes in rural communities.
About the Role
The Cardiometabolic Health Program Lead serves as the statewide leader responsible for coordinating and advancing Initiative 5 of the Rural Health Transformation Program (RHTP). This initiative focuses on improving cardiometabolic health outcomes through prevention, early detection, lifestyle interventions, and stronger integration between clinical care and community-based services.
This role blends strategic leadership with hands-on implementation support. You will design program models, guide regional execution, and ensure alignment with Indiana Department of Health priorities and CMS expectations. Working closely with IDOH leadership, Regional Grant Managers, and statewide partners, you will help ensure consistent, high-quality implementation across all rural regions.
As a visible leader within the initiative, you will connect policy, program design, and real-world delivery, ensuring that evidence-based strategies translate into measurable improvements in population health.
What You’ll Do
Program Design & Strategic Leadership
Shape the statewide vision and framework for cardiometabolic health improvement.
- Lead design and refinement of the statewide cardiometabolic health model, including prevention, screening, referral pathways, and community-based supports
- Develop standardized tools, templates, workflows, and implementation guidance
- Establish program goals, KPIs, and performance measures aligned with CMS and IDOH priorities
- Ensure integration with other RHTP initiatives, including workforce, care coordination, and data efforts
Regional Implementation & Technical Support
Enable consistent, high-quality execution across all regions.
- Provide technical assistance to Regional Grant Managers and implementation partners
- Support development of regional work plans and selection of evidence-based interventions
- Troubleshoot implementation challenges and provide real-time support
- Facilitate cross-region alignment, learning, and sharing of best practices
Clinical & Community Integration
Strengthen the connection between healthcare systems and community-based prevention.
- Develop and promote models linking clinical care with community programs
- Support implementation of screening protocols, referral systems, and follow-up processes
- Collaborate with statewide partners and associations to expand infrastructure and coordination
Data, Evaluation & Performance Monitoring
Ensure program impact is measurable, transparent, and continuously improving.
- Define metrics, data collection processes, and reporting expectations with evaluation teams
- Review regional performance data and identify opportunities for improvement
- Prepare summaries, reports, and presentations for IDOH leadership and CMS
Stakeholder Engagement & Partnership Development
Build strong relationships across Indiana’s rural health ecosystem.
- Engage hospitals, FQHCs, public health departments, EMS, community organizations, and academic partners
- Represent the initiative in statewide meetings, workgroups, and partner convenings
- Communicate program expectations and guidance in clear, actionable terms
Program Improvement & Innovation
Continuously strengthen the initiative through insight, adaptation, and forward-thinking approaches.
- Identify gaps, risks, and opportunities to improve cardiometabolic health outcomes
- Recommend programmatic, operational, or policy improvements
- Monitor national trends, emerging models, and evidence-based practices
What You Bring

Bachelor’s degree in public health, health administration, nursing, nutrition, exercise science, or related field (Master’s preferred)Required

Work Environment & Travel
- Hybrid role based in Indianapolis, Indiana
- 3 days onsite at the IDOH office in downtown Indianapolis and 2 days remote
- Full-time contract position with a standard 37.5-hour work week
- Regular participation in statewide meetings, workgroups, and trainings
- In-state travel required to support regional implementation and partner engagement
- Travel reimbursement provided in accordance with Indiana State Travel policy
