About the Opportunity:

Washington County Behavioral Health Division, within the Department of Health and Human Services, is recruiting for a Bilingual (Spanish) Adult Care Coordinator to join our team.

The Bilingual Adult Care Coordinator will serve adults with severe and persistent mental illness (SPMI) who are involved in several different programs, including licensed residential treatment, Aid and Assist, and occasionally civil commitment. This position will coordinate care throughout the residential care episode from the time of referral until the time of discharge. The care coordinator assists individuals under Aid and Assist orders by connecting them to services and supports that help stabilize symptoms and enable them to navigate the legal system successfully. This includes both individuals who are residing in the community and who are admitted to the Oregon State Hospital for competency restoration. Care coordinators collaborate with a variety of partners on behalf of clients, including hospital social workers, outpatient providers, health plans/CCO’s, legal system partners, housing system partners, natural supports, and more. They will meet with clients to assess system needs and create a plan to assist with system navigation. They will also attend and facilitate team meetings for clients to support transitions in levels of care, including discharges from the Oregon State Hospital or residential treatment programs. Occasionally, they will coordinate care for individuals who are civilly committed to ensure adherence to conditions of commitment and assist with access to services for stabilization during the commitment period.

Bilingual Positions: 

Designated bilingual positions require that the incumbent have knowledge of another language (primarily Spanish) besides English. It is required to possess precise writing skills (including spelling, grammar, and punctuation), fluent speaking skills, and the ability to communicate clearly and concisely both orally and in writing. Upon successfully passing the applicable proficiency assessments in a selected language, positions that are eligible for the bilingual differential pay will offer in an amount of 5% of the base pay rate, or a flat rate of seventy-five ($75) per pay period, whichever is greater. Pay periods are every two weeks. 

Work Schedule:

This position may be eligible for a hybrid work schedule including remote work.

Ideal Knowledge, Skills, and Abilities (KSAs):

  • Direct experience and knowledge of working with adults with severe and persistent mental illness.

  • Experience and knowledge in at least one of the following areas is preferred but not required:

    • Aid and Assist,

    • Behavioral Health Residential Treatment, and/or

    • Civil Commitment.

  • Experience and knowledge of behavioral health resources and systems in Oregon, including Medicaid/CCO’s.

  • Experience and knowledge of working with clients with dual diagnosis (Behavioral Health and SUD) is preferred but not required.

  • Understanding of the needs of communities with various backgrounds in Washington County.

  • Strong organizational skills and ability to prioritize and manage a variety of tasks in a fast-paced environment

  • Ability to complete accurate documentation in a timely manner

  • Strong collaboration skills and ability to build and maintain relationships with clients and system partners.

  • Compassionate and strength-oriented

Minimum Qualifications

Education and Experience:

  • A Master's degree in Counseling, Psychology, Social Work, or related field; AND three (3) years of increasingly responsible experience in providing case management or services coordination in a human services related field.
    AND

  • Must be certified by the Mental Health and Addictions Certification Board of Oregon as a Qualified Mental Health Professional or hold an approved clinical license and meet the criteria of a Qualified Mental Health Professional as defined in the Oregon Administrative Rules.

About the Department/Division:

The Behavioral Health Division has broad responsibilities as the local Community Mental Health Program (CMHP). This position will primarily serve adults with severe and persistent mental illness who are under Aid and Assist orders in the community or at Oregon State Hospital, receiving residential behavioral health treatment, and occasionally for individuals who are civilly committed.
