Job Summary and Responsibilities
As our Clinical Transformation Coordinator, you will develop, coordinate, maintain, and evaluate critical practice transformation initiatives focused on clinical quality, population health, and value-based care for the Physician Enterprise Central Region.
Every day you will collaborate across the continuum with leadership and management teams to ensure patient transformation supports key strategic goals. You will provide high-quality analytic support and technical expertise, leading or supporting complex projects requiring in-depth data evaluation to drive integrated care solutions.
To be successful in this role, you will demonstrate strong project management and analytical skills, a deep understanding of value-based care and population health strategies, and exceptional collaborative abilities to drive effective practice integration and patient transformation initiatives.
- Lead, coordinate, and/or participate in a variety of patient transformation initiatives, new care model redesign, and program operations to meet the highest standards necessary to meet quality and service goals. This could include working with grants, community, and value based care opportunities. Drives to achieve objectives, timelines and deliverables and to ensure results attained are consistent with expectations. Coordinate efforts to expand initiatives across the region.
- Build timely and effective cross-functional relationships with healthcare provider leadership and teams to interpret/explain clinical data derived from data management/analysis activities; establishes personal credibility sufficient to facilitate trust, understanding and acceptance of recommendations.
- Develop and deliver presentations to staff and other stakeholders to inform and promote projects and initiatives. Facilitate meetings and provide training related to initiatives. Develops, implements and evaluates standards, policies, procedures, training materials and/or general communications in support of quality systems/programs/processes.
- Utilizes recognized methodologies to turn data into information, deriving new data, integrating disparate data and providing reports to decision-makers; develops and implements management tools to display and facilitate understanding of clinical data.
**Must Reside in close to Omaha, Nebraska office**
