The Opportunity
The Care Coordinator is part of our amazing Amplar Preventative Health Team.
In this role you will work through a variety of service delivery modes –video and telephonic delivery or digital programs to support people with complex and chronic care needs in the community.
Drawing on your holistic knowledge and background in health, specifically chronic health management, you will support health literacy and empower participants to self-manage their health. You will utilise a coordinated and integrated approach, facilitating health behaviour change and care coordination in the delivery of health coaching.
This is a unique work from home role that allows you to be located anywhere in Australia. We can offer permanent and fixed term opportunities with full or part-time hours (minimum 0.6FTE). It is required to have suitable office area from which to work and reliable internet connection.
Our service operates between the hours of 08:00AM - 6:00PM (AEST) Monday to Friday (availability to work Friday is highly desirable).
About You
You are an advocate for preventative health and have the natural ability to build rapport with a wide range of clients and healthcare professionals. You have experience working in care coordination and delivery of support services to people with complex and chronic care needs.
You are a Registered Nurse or Allied health Professional (Occupational Therapist, Physiotherapist, Dietitian, Exercise Physiologist, Podiatrist and Psychologist) with relevant registration and experience with chronic disease management.
In addition, you will have the following experience:
- Care coordination and the delivery of seamless and effective support services to people with complex and chronic care needs.
- Experience in primary health care and care planning.
- Strong clinical knowledge and the ability to translate care plans into practice.
- Strong understanding of chronic disease management practices, including workflows across the care team.
- It is desirable but not essential that you have:
- Previous experience delivering care virtually and digitally, and
- An understanding of health behaviour change methodology and how this can be applied in practice.
