Job Summary
We are seeking aRemote House Calls Nurse Practitioner in NJ, OH, PA, NE, TX to support our in-home medical services program. This role providesvirtual clinical oversight, patient assessments, care coordination, and provider support for patients receiving care in their homes. The ideal candidate is comfortable practicing telehealth, managing complex chronic conditions, and collaborating with field-based clinicians.
This is a fully remote position with a strong focus on quality, efficiency, and patient-centered care.
Responsibilities
- Conducttelehealth visitsfor patients enrolled in the House Calls program
- Manage chronic conditions such as diabetes, hypertension, CHF, COPD, and geriatric syndromes if the patient requires primary care services
- Performcare management needs assessments, follow-ups, and care plan adjustments
- Review patient histories, labs, imaging, and diagnostic results
- Supportmedication management and reconciliation
- Coordinate care with primary care providers (if patients already have one), specialists, and care management teams
- Document visits and clinical decisions accurately in the EMR
- Ensure compliance with clinical protocols, quality standards, and regulatory requirements
- Participate in interdisciplinary case reviews and quality improvement initiatives
Qualifications
- ActiveNurse Practitioner (NP) license in the required state(s)
- National NP certification (ANCC or AANP)
- Minimum2+ years of clinical experience as a Nurse Practitioner
- Experience inhouse calls, home health, geriatrics, primary care, or internal medicine strongly preferred
- Priortelehealth or remote care experience preferred
- Strong clinical judgment and comfort managing complex patients remotely
- Excellent communication and documentation skills
- Must be licensed in NY and one of the other states listed or willing to obtain
Technical & Regulatory Requirements
- Proficiency withEMR/EHR systems
- Comfortable using telehealth platforms and remote monitoring tools
- ActiveDEA registration (or willingness to obtain, if applicable)
