SUMMARY: Provides telephonic triage and document review services that require clinical judgment and interpretation and acts as a liaison between the patient, care team(s), and their primary care provider. RESPONSIBILITIES: Reviews and triages telephone encounters, calls, and messages from patients and de-escalates or escalates to the appropriate care team or clinicianPrioritizes urgent medical triage to patientsProvides timely assessment for smart scheduling into clinician scheduleReviews lab results, imaging results, consultation notes, and clinical summaries and identifies those that need to be directed to the respective clinicians for reviewEffectively communicates with appropriate disciplines involved in the care of patients, and provides information and recommendations to providersDevelops professional working relationships with health care providers and facilities, generating an open flow of information and support of home care goalsPromotes effective working relationships with nursing personnel, the management team and health care providersDetermines individual patient needs for services, referrals and training and provides patient education as necessary on plan of careAttends required meetings and huddlesSeeks guidance and resources, when necessary, in care and treatment of patientsProvides guidance and direction to colleagues and acts as a resource/mentor for nonclinical staffCoordinates team based care in the absence of the Program ManagerComplies with federal and local confidentiality laws, including HIPAA, ensuring patient privacyAdheres to Brown Health Medical Group Primary Care guidelines and policies for protecting patients’ demographic, clinical and financial informationPerforms other job-related duties as assigned

Pay Range:
