Position Overview:

We are seeking Florida Licensed Medication Therapy Management (MTM) Pharmacists to join our remote team. In this role, you will be responsible for conducting outbound calls to perform Comprehensive Medication Reviews (CMRs) and provide in-depth consultations on statin medication use in accordance with American Heart Association (AHA) guidelines. You will transcribe patient-specific prescription and diagnosis information into a third-party, web-based application to generate standardized documents for patient communication.

Key Responsibilities:

• Conduct up to 100 outbound calls per day to complete CMRs.

• Deliver MTM services with a focus on SUPD, SPC, and CMR Star measures.

• Accurately transcribe clinical data into a web-based platform.

• Ensure timely and confidential handling of patient information.

• Collaborate with the MTM product team to support patient care initiatives.

Qualifications:

Education & Licensing:

• Must reside in Florida

• Must possess a Valid Florida Pharmacist License

• Minimum 4 years of clinical MTM pharmacist experience (management experience preferred).

• Proven track record in driving Part D Star measures.

• Clinical experience in medication review is essential.

Skills & Competencies:

• Fluent in spoken and written English.

• Excellent verbal and written communication skills.

• Proficiency with Salesforce is a plus.

• Must be tech-savvy and comfortable with digital tools.

 

Pay Rate- The anticipated wage for this position is between $  $58.00 to $59.85

Hourly wage depends upon experience, education, geographic location and other factors.
