Job Title: State Services Coordinator- Region 1 Field Office- Cumming

Pay Grade: I

Workplace: Remote

Travel: Regular travel within assigned counties: Banks, Dawson, Forsyth, Franklin, Habersham, Hall, Hart, Lumpkin, Rabun, Stephens, Towns, Union, and White

Job Summary:

The candidate chosen for this position will perform the duties listed below in multiple counties in Georgia. The

position is field based, with supervision from the Regional Field Office.

State Service Coordinators develop service plans with and monitor services for Individuals with intellectual

or developmental disabilities (IDD) who have been screened and determined eligible for State Funded

Services. State Services Coordinators are assigned specific counties/provider locations, and under general

supervision, will perform the following duties for Individuals on their caseloads:

Communicates with individuals in a manner appropriate to the individual's abilities to gather required information; communicates with families and other authorized parties to obtain information necessary to maintain awareness of changing circumstances, current level of need, and specific support needs

Assist individuals with IDD to choose state funded service providers by providing information on providers and facilitating meetings with potential providers

Periodically reassess needs, hopes and desires coupled with assessment of risks and protection

Schedules and facilitates ISP meetings

Develops Individual Support Plans (ISPs)

Develops and maintains knowledge of community resources and maintains a network of community contacts; uses this knowledge to educate and advise families, providers, and caregivers as appropriate

Complies with HIPAA and all DBHDD policies regarding confidentiality of protected health information

Reports concerns of abuse, neglect, exploitation or immediate risk to an individual as required by policy

Systematically monitor the implementation and the effectiveness of the ISP and the health and safety of each individual on the caseload via visits and review of documentation

Preferred Qualifications ‐ External:

Experience with or knowledge of individuals with intellectual/developmental disabilities, Medicaid waivers, and DBHDD I/DD services; experience working successfully without daily contact with a supervisor.

Bachelor's degree in a related field from an accredited college or university and two (2) years of job-related experience; or two (2) years of experience at the lower level Social Svcs Prgm Coord 1 (SSP030) or position equivalent. Note: Some positions require eligibility for licensure.
