Nuna Community Health Workers play a vital role in supporting community wellness by combining traditional CHW responsibilities with the innovative use of the Nuna App. They engage with and educate community members while also promoting the goals and functionality of the Nuna app. CHWs conduct comprehensive needs assessments, address social determinants of health, and connect individuals to partner-approved resources. They collaborate closely with care coordinators and healthcare providers to ensure members receive holistic support. In addition, they maintain strong relationships with provider care teams and facilitate access to additional services as needed, helping Nuna App users navigate their care journey more effectively.

This role demands a highly motivated and empathetic individual, capable of multitasking in a dynamic environment and committed to improving the health and wellbeing of local communities.
Educational and Supportive Responsibilities

Educating members on preventative care

Providing virtual support for members' health needs, including short-term interventions and collaborative care management in conjunction with our health care partners

Conducting proactive outreach and maintaining engagement with members, especially those with high needs, to help them achieve their health goals

Documenting member interactions and organizing this information efficiently
Referral and Assessment Responsibilities

Making necessary referrals to the care team for additional medical or social resources

Conducting thorough needs assessments with patients in various settings and assisting with health-related social issues

Helping patients set and reach health goals, addressing social determinants of health, and assisting in patient coaching
Coordination and Community Outreach Responsibilities

Leveraging a provider-created and approved directory of local community resources

Managing relationships with healthcare facility providers and working closely with the care team

Serving as a liaison between services, aiding in care coordination, and advocating for clients

Attending community events for outreach and education
Technology and Documentation Skills Responsibilities

Teaching patients how to use the Nuna app and providing critical feedback from the field to the Nuna product team

Navigating various technology systems for detailed and accurate documentation of patient encounters
QUALIFICATIONS
Required Qualifications
-
Master’s Degree in Social Work, Public Health, or equivalent experience

Proficiency in using technology for remote support and comfortable with virtual communication platforms

Strong problem-solving skills with a creative approach to healthcare challenges

Adaptability to change, excellent organizational skills, ability to work independently

Strong communication skills, cultural competency, and a commitment to serving diverse and underserved populations

Familiarity with the Greater Chicago area and its community resources with residence within commutable distance of Chicago and willingness to travel within surrounding counties
Preferred Qualifications

Community Health Worker certification

Experience with managed care patients and roles in customer or client service

Knowledge of Chicago Medicaid populations
