Job Summary:
The Community Health Worker's role serves as the liaison between health care providers, care coordination, and community engagement to facilitate service delivery within identified communities. Community Health Workers connect with patients in the comfort of their homes or at designated community locations, offering resources and fostering a strong sense of community support. Some occasional night or weekend work.
EXPERIENCE DESCRIPTION:
A minimum of 1 year of experience working with diverse communities coordinating and facilitating service delivery is required.
Experience preferred: working as an interpreter, significant experience navigating life as a newcomer to the US.
Preferred language skills: include Rohingya, Burmese, Swahili, and Somali
EDUCATION DESCRIPTION:
High School diploma or equivalent is preferred.
TRAINING DESCRIPTION:
Completion of Community Health Worker Program through accredited organization (Sponsored by Froedtert Health if no prior training upon hire. Must Be Completed within One Year of Employment at Froedtert Health).
