Title: Care Associate (Healthy Beginnings Program Care Associate)

Job Description:
Position Summary:
The Healthy Beginnings Program Care Associate tracks and supports a cohort of assigned enrolled patients supporting them from prenatal care through the infants first year of life. Supports patient adherence to prenatal visits, their infants well baby visits, social and support services, and supports special events such as baby showers. Conducts patient screenings, home visits, and hospital visits as assigned.
This role is hybrid with 4 days onsite and 1 day remote.
Minimum Qualifications:
- Associates Degree in Human Services, Health Service Administrator, Community Health, or related field of study is required.
- 1-2 years of direct experience working with care navigation, community health, or related health-care discipline is required.
- Previous experience supporting maternal/child health is a plus!
- Excellent interpersonal skills including over the phone, in person, and e-mails.
- Computer skills including Microsoft Office, Data-Driven software programs, electronic records, etc. are essential.
- Bilingual Spanish/English is required.
- Thrives in fieldwork driven role with comfortability with facilitating home visits for the program.
Key Duties:
- Participate in the identification and enrollment of OB patients seen at health center who are deemed high risk based on psycho-social screening and other risk factors.
- Provide care coordination support and track assigned enrolled patients from prenatal care through the infants first year of life supporting full engagement in prenatal and pediatric care under administrative and clinical leadership.
- Participate in team meetings and prepare the agenda for prioritized case discussions
- Interface with team members both internally and externally as permitted by HIPAA to coordinate services.
- Escalate cases to the Coordinator, Social Worker and Provider based on emerging risk factors.
- Attend training on administering select screenings and conduct screenings as assigned.
- Conduct home and hospital visits to ensure continuity of care for mother and infant as assigned.
- Lead patient recruitment and support engagement events such as baby showers, arts events, mommy and me classes, healthy weight post pregnancy classes, and healthy cooking classes as assigned.
- Complete documentation in the electronic health record as required
- Participates in the collection of program data for evaluation.
Work Environment Type, Location, and Schedule:
Hybrid, Bronx, NY, Monday - Friday 8am to 5pm and/or 9am-6pm.
Compensation:
Minimum: $50,000 annually; Maximum: $50,000 annually
