Position Summary: The Program Manager, HONEY (Housing Our Newborns, Empowering You) reports directly to the Program Director, HONEY. This position is responsible for the supervision of the HONEY Perinatal Care Coordinators and Perinatal Navigators, and Housing Perinatal Care Coordinator.

Highlighted Duties and Responsibilities:
- Helps to develop and manage partnerships with Department of Human Services (DHS) and Virginia Williams Resource Center (VWRC) staff to coordinate processes and integration for handoffs, data collection, coordination and education.
- Educates partners, clinicians and housing system staff on issues of perinatal health to support families during pregnancy and infancy.
- Coordinates closely with COH housing staff and other housing agencies and advocates to ensure appropriate care and effective collaboration. Maintains a clearinghouse of appropriate and preferred community resources for perinatal care, in coordination with housing case managers.
- Help to develop and maintain working partnerships with community healthcare providers offering perinatal services.
- Meets with supervisees regularly and provides real-time feedback, training, support, and coaching as needed. Creates meeting agendas, sends meeting agendas in advance when possible, and documents concerns as needed.
- Monitors use of assessments to evaluate risk factors and connection or barriers to receiving perinatal services, preferences for care, potential risk factors, and understanding of an preparation for birth and infancy.
- Reviews data in Quickbase, EClinicalWorks (eCW), CRISP and other resources to ensure that staff are adequately capturing activities for care coordination and program evaluation at least twice a month and more often as needed.
- Trains and supports staff in client-centered techniques for providing information to clients on the importance of perinatal health care, with an emphasis on increasing understanding of healthy pregnancy and warning signs, nutrition, self-management of care and primary care.
- Takes initiative and makes decisions within the scope of work. Assesses situations independently, thinks critically, and takes action to address issues or make useful recommendations while maintaining a solution-oriented lens.
- Completes other duties as assigned by supervisor.
Requirements
- LGSW or LICSW or equivalent experience preferred.
- For Bachelor's of Social Work (BSW) degree or Master's of Social Work (MSW) degree holders, the appropriate DC license must be obtained prior to start date.
- Experience providing social services, health care coordination or health care education required.
- Familiarity with perinatal services and standard of care required.
- Ability to travel between sites required.
- Experience working with under-resourced population, including pregnant people experiencing homelessness strongly preferred.
- Familiarity with Medical Assistance system and community resources strongly preferred.
- Strong organizational skills.
- Strong written and verbal communication skills.
- Demonstrated ability to collaborate effectively in a team setting.

- In relation to remote work versus on-site expectations, this position is classified as: Hybrid - Remote work and on-site work requirements are split roughly 60/40. Please note that remote work designations are subject to change or fluctuate at any point in time and the supervisor may require in-person learning for a specific amount of time after hire.

About Us:
Community of Hope is a mission-driven, innovative, and rapidly growing nonprofit. For 45 years, we have provided healthcare, housing, and community support services to make Washington, DC more equitable. As a Federally Qualified Health Center, we provide medical, dental, emotional wellness, and care coordination services for the whole family at three locations in DC.
Community of Hope also strongly emphasizes maternal and child health, with midwifery practice and the only free-standing birth center in DC. In 2024, Community of Hope provided about 50,000 medical visits, 6,300 dental visits, and 17,000 behavioral health visits for about 16,000 patients. Community of Hope is also one of the largest providers of homelessness prevention and housing services for 1,600 households—1,384 families and 220 individuals—experiencing homelessness in DC.
