Summary:
The Health Training Specialist promotes, maintains, and improves individual and community health by providing health coaching to patients across the service area. Working under a Supervisor, this role is responsible for delivering the Diabetes Prevention Program and the Living Well with Diabetes Self-Management Program to participants in a virtual, group setting (per diem). The Health Training Specialist is trained to deliver program curriculum, document patient interactions in the medical record, and support effective patient communication.
This is a remote position that leads classes during evening hours (5-9 p.m.) on Tuesday, Wednesday, and/or Thursday. Hours may vary based on program needs and patient demand. Onboarding will require daytime availability for training.
Required Minimum Knowledge, Skills, and Abilities (KSAs)

Education: Bachelor's degree in health education, Public Health or a closely related field preferred.

License/Certifications: Certified Health and Wellness Coach preferred.

Experience: Experience in nutrition and/or physical activity preferred.
