Therapy Care Specialist – Remote (United States)

Soleo Health is seeking a Therapy Care Specialist to work remotely. The Therapy Care Specialist works with SoleoRx and utilizes national and local resources to facilitate high quality care of a patient with specialty pharmacy needs. Therapy Care Specialists will work under the supervision of a Pharmacist to oversee logistics of a patient’s dispense and prescription maintenance, assessment data capture, and serves as the point person to coordinate patient care across all layers of the patient care continuum while on service with Soleo Health

ESSENTIAL DUTIES AND RESPONSIBILITIES

  • Act as single point of contact for specialty patients and as liaison with Soleo branch personnel to coordinate timely medication dispense and delivery
  • Works under supervision of specialty pharmacist and/or branch pharmacist to input new prescription data, refills, and capture assessment data
  • Manages patient schedule for dispense, delivery and assessment
  • Run daily and weekly pharmacy reports as well as keeping the pharmacy logs for the state
  • Answer phones and warm transfer call to appropriate location, monitor incoming faxes, monitor and upkeep of the T-drive and CPR+ documentation
  • Send orders to physicians for signature and keep the home health agency up to date
  • Scan in daily documents, prescriptions, delivery tickets or other document management as needed
  • Adjudicate claims, complete insurance overrides, and work with intake/reimbursement staff to maintain active patient insurance coverage
  • Documentation of all patient interaction in the designated system of record
  • Complete therapy specific training as required
  • Other duties as assigned

QUALIFICATION REQUIREMENTS

  • High School Diploma or equivalent. Some college credits preferred
  • Pharmacy Technician Licensure in SoleRx’s home state and state of Therapy Care Specialist’s Residence
  • Registered/Certified Pharmacy Technician (CPhT), or as required by the BOP
  • Prior experience in home infusion and/or specialty pharmacy preferred
  • Current registration and ACPE requirements are met
  • Excellent customer service skills
  • Basic to intermediate efficiency with Microsoft Excel, Word and Outlook.
